THIS FORM IS ONLY FOR TOURNAMENTS OCCURRING AFTER FEB. 18th, 2003.  CORRECTIONS WILL NO LONGER BE ACCEPTED FOR TOURNAMENTS BEFORE THIS DATE.  FOR MEMBERSHIP INFORMATION UPDATES (incorrect spellings of names, state/address changes, etc.), PLEASE CONTACT ratings@usatt.org.

Please ensure that you include the following information for your correction.  We will not consider any request that does not follow these guidelines:

Player Requesting Correction
Contact (email or phone)
Tournament Name
Tournament Date
Event Name (if it is shown on the results pages)
Explanation - list players involved and what the correct result should be.
