USOC is requesting that all potential candidates for the 2007 Pan Am Games to register on line and fill out the following documents:
Athlete Biographical Questionnaire
Athlete Medical History Form
Games Forms Final
HIPPA Form 

 In addition to these forms the potential candidates will have to submit:

One digital headshot submitted electronically in JPEG format (30-100 KB) Note: The background of this photo must be light in color (white or light gray).  Hats, headbands, and dark glasses may not be worn in the photo.  Please no funny facial expressions!

After you register on line at: https://events.usoc.org/PanAm07Registration/, fill out, sign the forms and fax, email or mail them not later than February 10, 2007 to:
